And now... the promised tutorial for macOS, which also comes with a
self-installer package for this platform:

Using the self-installer package, bootstrapping a pkgsrc installation
on macOS and rolling custom packages are trivial operations. Hopefully
this helps someone other than me, and helps in getting more pkgsrc
users on the Mac!
