I tracked it down to change from May 13, 2008. (After many tests, I found 
if I reverted src/sys/arch/x86/x86/intr.c back to 1.49 I can boot again.)

(Now I see a followup on that ticket from Feb. 2009, somehow I missed 
that. Looks like that second problem is related to
