That probably says that there is no formatted filesystem,
neither FFS (very likely) nor FAT (that would be common)
on partition 'a'.

That doesn't agree. While a FAT partition at block 2048 on a 16GB drive
isn't that common, it's probably correct.
A synthesized disklabel would match the MBR, so that one has probably
been written to the USB drive.
Can you try to erase the label with 'disklabel -D sd0' ?
