I may have narrowed it down to the USB device. Basically i can place the
USB device in a state such that any operations on /dev/sd0e produce an
"Input/output error" until i manually remove the USB stick from the USB
port and then re-insert it.

If after doing the vnconfig .... i cause the problem that generates a
"Input/output error" and then i go about manually cleaning up,
unmounting everything, un-configuring the vnd device but leaving the USB
stick mounted. If i then try and read from any of the files on the USB
sticks mounted dos filesystem, i get "Input/output errors" then too.

Then i unmount the USB dos filesystem: umount /mnt/flash
and try to mount it again: mount /dev/sd0e /mnt/flash
I get the error:
mount: cannot open '/dev/sd0e': Input/output error

Finally, if i remove the USB stick from the USB port and then re-insert
it. I can use it successfully again for a while.

There may be some pattern about the usage of the compressed vnd device
that causes this problem in the USB that i have not been able to
reproduce with a non-compressed image.

I have attached some logs that may be helpful from:
