Is this reproducible or has it happened just this once?  It seems like
you were installing a package from the top of the screenshot.

Can you please file a bug for this with send-pr?  Any extra details
about what you were doing at the time will be helpful.  For example, was
this a bulk package install (assuming that it was a package install at
the time)?  Which package was being installed?  Did you start from an
empty /usr/pkg or updating existing packages.
