vga1 at pci5 dev 0 function 0: vendor 0x10de product 0x0142 (rev. 0xa2)

That should be
  BusID "PCI:5@0:0:0"
in the xorg.conf

Does libpciaccess handle a vga card at pci5?

You could try to use the XF86Config.new and change the BusID.
