Not an add-on card, so may be off topic, but

  mpt0 at pci1 dev 0 function 0: vendor 0x1000 product 0x0058

is working for me with 2 SAS disks appearing as a

  sd0 at scsibus0 target 0 lun 0: <Dell, VIRTUAL DISK, 1028> disk fixed
