probably best to make uvm_setpagesize() and other now-internal
functions not part of the global UVM api.  ie, move them to
uvm.h and remove the docs in the man pages.  at this point they
don't need to be documented in the uvm(9) manual.

(hmm, actually, that manual says to use uvm.h, though most of
the kernel should be using uvm_extern.h.)
