I haven't heard anyone in this thread mention pkgin yet. It works with 
binary packages. It probably solves the problems for most users. During a 
normal upgrade: "If the installed dependencies match the listed needed 
dependencies, don't upgrade them."

(Yes, a fault with pkgin is that it and its dependency is not in the 
default install.)

Who uses pkgin? What could it do better? Or what is it missing?

(Missing for me is package repo built using X11_TYPE=modular. Also I have 
a patched pkgin that provides debugging/dry-run feature, better error 
reporting, and more verbosity.)
