patches/patch-am introduces code to look for ndbm.h which has been copied from 
further up in setup.py, but the filename variable there hasn't been fixed to 
"ndbm.h" for the OS X special case.

this uses the same fix for all python versions, but note that I have *only* 
tested this with python27.
