This has been fixed with rev. 1.11 of sys/arch/x86/include/i82489reg.h.
Tested with NetBSD-current under CentOS 6.2/kvm with 24 virtual cpus.

There is a message "ioapic0: can't remap to apid 24" which seems harmless.

Requested pullup to NetBSD-5 (along with hpet timer and virtio patch).
