The man page is part of the package (it's written in old classic man,
not mandoc), so it is better handled upstream.

Probably, but that doesn't matter too much.

This should be supplied by the program. The man page says:
| The allow_severity and deny_severity variables determine how
| accepted and rejected requests may be logged. They must be
| provided by the caller and may be modified by rules in the access
| control tables.
So I'd really say all these are not bugs.
