This problem occurs on a Gigabyte GA-EX58-Extreme motherboard when the support
for Legacy USB keyboards is disabled in the BIOS which is the default setting.
Enabling Legacy USB keyboard support solves the problem. This isn't a problem
once the kernel is booted up and the keyboard works fine at that point
regardless of the BIOS configuration. It appears the root cause of this problem
is the understandably limited USB support in the boot code and the BIOS
defaults on the Gigabyte motherboard(s). The PR can probably be closed,
although it may be helpful to note this issue in the installation documentation.
