Just for the record: I got it working.

Various bugs on my side, and I needed to order the various "map" statements
in my npf.conf from most specific to catch-all default NAT map.

I also tried with pkgsrc/net/siproxyd, which might have been a cleaner
solution theoretically, but I couldn't get the fritzbox to properly talk
to it (stupid UI), and I would have lost encryption.
