Pull up following revision(s) (requested by bsiegert in ticket #1397):

        sys/arch/amd64/amd64/locore.S                   1.122 (via patch)
        sys/arch/amd64/amd64/machdep.c                  1.254 (via patch)
        sys/arch/amd64/amd64/trap.c                     1.95 (via patch)

Remove the call gate on amd64, it is useless and vulnerable.
