Log Message:
Rewrite dtrace lockstat from the Solaris version and simplify.

- Fix mutex_adaptive probes to use the right lockstat flags.
- Install lockstat_probe_func on module load, not trace enable.
- Use a more sensible memory barrier arrangement.
