- Replace simple_lock/ltsleep/wakeup with mutex/cv.
- Replace malloc/free with kmem_alloc/kmem_free.
- Use bdev_ioctl() instead of bdevsw_lookup()/(*d_ioctl)().
- Be more fair to readers by cleaning the cache one slot at a time.
