Pull up following revision(s) (requested by pooka in ticket #535):
        sbin/mount_procfs/mount_procfs.8        1.28-1.33
sync file list with reality
add a description for fd/# (with reference to fd(4))
proc/#/exe is now exposed for all mounts, move it away from linux-only
