Pull up following revision(s) (requested by bouyer in ticket #151):
        sys/dev/pci/bktr/bktr_core.c: revision 1.38
        sys/dev/pci/bktr/bktr_os.c: revision 1.43
        sys/dev/pci/bktr/bktr_core.h: revision 1.8
stop attaching when a bus_dma operation fails, instead of using bogus
addresses.
Remplace vtophys() with proper bus_dma(9) use.
This driver now works properly on Xen
