* Use uint{8,32}_t from <netinet/in.h> where applicable; use private
  fixed-width integer types otherwise.
* Protect RFC 2292 prototypes, which are not XNS5.2/POSIX-2001; also, define
  size_t for inet6_rthdr_space().
