Add a lot of byte swapping to get the code to work on big endian machines.
Patch from Tsubai Masanari <tsubai%iri.co.jp@localhost>.
Add an extra controller reset.  According to the OHCI spec it shouldn't
be necessary, but without some machines don't work.
