I think in reality no one goes for core approval for anything that isn't
a major system architectural change, and often it isn't done explicitly
even then.

Adding ioctls to a driver is not major enough to require high level
approval. In fact, if you "own" the driver or subsystem, often it
doesn't even require discussion.
