It's pretty definitely faulty. I tried booting 4.0.1 and that fails to
fully recognize the Phobos card as well. The Challengs S with the faulty
board had previously been running Irix, but without any expansion cards,
so hadn't shown any symptoms. Unfortunately I don't have an Irix system
with either the Phobos or Set drivers to test with. The mezzanine card
in both my Challenge S machines is the same revision:

SCSI-MEZZ CHALLENGE-S 030-8221-002 REV D

I've included the boot messages for 4.0.1 and 5.0 on the faulty board
below. Under 5.0, the Ethernet interface on the mezzanine board works
fine so the fault must be fairly specific to the GIO expansion sockets.
5.0 still hangs on R5000 though :(
