Another update.  I ran some more experiments.

I pulled the Promise ATA133 card, and ran with a single disk attached to the
internal ATA bus.

In this configuration, if I run "newfs /dev/rwd0f" in a loop, within
about 8 iterations, it always hangs.

I tried it with both the stock and the no-ATI kernel.  Same result.

I am now trying the same thing with a 2.1 stock kernel.  I'll leave the
loop running overnight......
