I have used editors/xournal for that kind of thing, though it is  bit
flakey (or has been sometimes).

It generally keeps the modifications in a separate (non-PDF) file, in
which form they can be modified, removed, etc, but it cn export the
combined thing into a new PDF file (so apps like xpdf can see the
annotations.)
