While upgrading a database server with binary packages, I wanted to switch from 
MySQL 5.1 to 5.5. The package mysql-server-5.5.32nb2 installs correctly, but 
its rc.d file refers to ${PREFIX}/bin/mysqld_safe which is not installed by the 
package. Note that this file is not present in package's PLIST either, while it 
is in mysql-server-5.1*.
As expected, the service fails to start.

This affects pkgsrc-2013Q2 at least.
Install mysql-server-5.5* binary package and try to start MySQL using 
/etc/rc.d/mysqld start
Possibly adding mysqld_safe to PLIST, not sure about it.
