Added a proper header comment that lists some of the common variables
and targets.

Removed handling of DOCS and the "orphans" target, since that has been
imported in revision 1.1 and apparently never been used.

Removed the implicit handling of files using .SUFFIXES, enforcing all
interesting files to be listed properly in the Makefile.
