- Changed indentation and whitespace. Since this file does not contain free
  text, I think we should keep all <tocentry> elements on a single line.
- Removed redundant "dir" attributes ("dir" attributes are cumulative
  within the hierarchy).
- Inserted some necessary "dir" attributes.
- Sorted entries (Ports/mmeye).
