Update for 2.0:
 * rewrite some texts for better readability
 * xref manpages
 * rearrange sections to first describe inetd.conf, then the test
 * document hosts.{allow,deny}
 * when giving example commands, also show some output, so people
   reading this not in front of a terminal get some idea of what the
   commands print
 * condense links/etc. section into a single secton w/ no subsections
