- make it clear the meeting was held in 2004
- use <quote> instead of "s in a couple of places
- add missing trailing / on some URLs
- s/Digital Unix/Digital UNIX/
- strip some trailing dots from <itemizedlist>s for consistency
- <ulink> doesn't need any content if the content is the same as the
  url
