convert all (4) HTML 2.0 character entities to proper syntax, instead
of just doing < and > (sometimes), then convert < and > back if they
match, and convert " and & back if they exist within a <...> pair.
This will make it easier to do proper conversion of <> based on the
tag(s) that they may or may not contain...
