The pkg and pkgsrc tools should create the directories needed.

For example, on one system I have:
 bin/      etc/      info/     libdata/  man/      sbin/
 emul/     include/  lib/      libexec/  pkgsrc/   share/
(I put the pkgsrc directory there.)
And on another:
 bin/      emul/     include/  lib/      libexec/  pgsql/    sbin/
 certs/    etc/      info/     libdata/  man/      private/  share/
