How about prompting the user, which can be overriden by an environment
or mk.conf variable?  This will obviously only ever happen once on a
system, so it shouldn't be a burden...

Perhaps a later version of sysinst can also set up /usr/pkg (or /pkg?
where did that thread finish?) as well.
