the MAP_NOEXTEND flag appears in sys/mman.h, but not in the mmap(2)
man page.  in fact, sys/mman.h is the only place i can find it in the
entire source tree.  it appears to be a vestigial part of the old mach
vm code, though i can't see that it was used much there, either.

unless someone can show good cause, i'll remove it.
