I think I have encountered a problem where the kernel doesn't recover
from this lack of perfection. I have a GSmart 350 for which I have
written a userland program that uses ugen. If I do something wrong in
the program, then the camera goes wonky and the USB subsystem will not
reset the port, even if the camera is removed from the computer.

I haven't mentioned this before because I don't yet have a test case
that clearly shows this behaviour. At the moment, this is mostly
guesswork on my part.
