Pull up following revision(s) (requested by skrll in ticket #273):

        sys/uvm/pmap/pmap.c: revision 1.91

Avoid NULL pointer de-ref in pmap_enter, and mark mdpp as
VM_PAGEMD_REFERENCED as intended in the __HAVE_PMAP_PV_TRACK
case.
