Major mode format restruction:
- Remove gv->hblank_stop and gv->vblank_stop
- Add gv->disp_flags to select interlace, doublescan, sync-on-green and
  horizontal/vertical sync polarity.
- Add many checks for old and wrong modes.
- Use a TNF copyright, with the permission from Ezra Story.
