* Re-direct I/O from to our vnodes at splbio(), and use vwakeup() instead
  of an expansion of that code.
* Make swapping on VREG files work for the simple one-page-at-a-time case
  (thanks to Jason for pointing at ready-to-use code from vnd.c).
* Have the "master" resource map simply use the range [1..INT_MAX]
