pg->phys_addr > VM_PAGE_TO_PHYS(pg)

Explicitly cast pointers to uintptr_t before casting to enums. They are
not necessarily the same size. Don't cast pointers to bool, check for
NULL instead.

vmx_vmptrst(): only used when DIAGNOSTIC

Simplify, remove unnecessary #ifdef DIAGNOSTIC around KASSERTs.

Use ULL, to make it clear we are unsigned.
