description:
* As is customary, append the parameter names from
  SYNOPSIS to the description of the functions.

* Use .Ss for non-standard subtitles.

* Fix two identical subtitles.

* Add missing .It's, fix few .Ft's.

* Use a little more markup.

* Improve offsets.
