pvgrub

If anyone has set up pvgrub with a NetBSD dom0 and used it successfully,
please post details.

The basic concept is to build grub (grub2 these days) with a target of
xen/pv, and to specify that as the kernel for a PV or PVH domU.  It then
reads grub config files and loads the real kernel.  The point is that
the real kernel is obtained from the domU filesystem and under the
control of the domU admin.

(It seems possible to add support for PV to /boot, but it also seems like
a lot of work.)
