The VAX pthread code was very broken.  After my commits today, it's
now just slightly broken.

I had to fix the kernel upcall code.  And then fix makecontext in
libc and the VAX specific bits in libpthread.  Most of the tests
in regress/lib/libpthread actually pass now.

However there are still problems.
