Your fisk disk controller should be set to the address of
17772150/20001468. I'd guess in your another 4000 there were other
disk controllers so the CQD was set to the secondary or thirdly
address. I guess the CPU might not scan the secondary or thirdly
addresses if it does not find a controller at the first address.

I have a CQD-440/443 manual. I don't know whether 223 and 440 share
the same jumper settings. But here is the CQD-440 jumper settings for
disk/tape addresses:
17772150/20001468: sw1-1 ON sw1-2 ON sw1-3 ON sw1-4 ON sw1-5 ON (this
should be your disk setting)
17774500/20001940: sw1-6 ON sw1-7 ON sw1-8 ON sw1-9 ON sw1-10 ON (this
is your tape setting).

"CQD-440/TM  supports disk and tape drive simultaneously"
