Did the disk and CDROM appear in the autoconfg output of the kernel at
boot time? 
Or exit the install system and do a 
dmesg
or 
cat /kern/msgbuf
to see the messages.

In general: It is best to do the installation by hand. See the mailing
list archive at http://mail-index.netbsd.org/port-vax/ for mails from
me with "installation problems" as subject. I described the hand
installaton procedure several times bevore...
