Several things to do and see:

First, verify the Firmware revision! Make sure there is an EPROM plugged in 
where the firmware goes :-) And verify whether or not it controls Tapes, 
Disks, or Tapes & Disks (Firmware /T, /M, and /TM) if it is tape only and 
it thinks it is the first Q-bus tape drive then it could be "hidden" by the 
TQK70.

Second you can try pulling _every_ board out except the CPU, Memory, and 
the CQD-220. Then do a 'SHO QBUS' to see if it shows up.

Third, if this is the 3300/3400 box, make sure this dual wide card is in 
the *TOP*, there are no qbus slots on the bottom only "C/D" slots. DEC used 
to provide little black plastic inserts that could support the card (common 
on the TQK70).

Fourth, make sure your terminal is set for 9600 baud, 8 data bits, one stop 
bit, no parity.

These cards do work with this stuff and NetBSD does recognize and boot from 
them.
