For the record, I just booted using netbsd.generic.gz "updated to
include the disconnect fix for the NCR SCSI driver" from 
ftp://ftp.netbsd.org/pub/NetBSD/arch/vax/snapshot/1.4.1-pre/kernels/
(yes, I checked the md5 hash) and am seeing the same thing:
ncr0: no REQ for next phase, aborting
when probing the SCSI bus.
