Just a couple of shots in the dark here, but does the drive show up okay
when you run a "show dev" from the VAX prom prompt ">>>"?  You may want to
also verify that the drive's id doesn't conflict with that of the SCSI
controller (which I've seen set sometimes to 6 or 7). The show dev command
will let you see the SCSI id info for all the devices on the SCSI bus.

And then perhaps you could have the VAX run a low-level format on the
RZ56.  The prom command "test 75" brings up a SCSI disk formatter.  Okay,
so maybe that's just voodoo, but it has some potential.

If you're using the 1.4 miniroot.fs, then that would most likely be the
source of you problem.  That miniroot doesn't contain a kernel with the
disconnect hack to make PIO SCSI support happy.
