If a 'dd if=/dev/zero of=/dev/sdxx' and 'dd if=/dev/sdx of=/dev/null' works 
then I start suspecting your RAM. Perhaps it (usually) gets in the FS
buffers or similar..

Just a shot in the dark, but it is something that happened to me before..
