I have:

bank0: 4MB   (00000000-003fffff)
bank1: 16MB  (01000000-01ffffff)
bank2: 16MB  (02000000-02ffffff)
bank3: 4MB   (03000000-033fffff)

An interesting datapoint is that this works fine with the 1.4.3 boot floppy
and Solaris 2.6 CD.  I'll try swapping bank0 and bank2 and see what happens.
