This is definitely looking like fallout from the R5K problem with L2
cache.

I see that you're booting a 1.5ZA kernel from the 20020307 snapshot
I generated.  Try the kernel from the 20020409 snapshot.  It has the 
L2 cache disabled.  The 20020307 snapshot wasn't tested on my end on
R5K's, and appears to have L2 cache enabled in the GENERIC kernel.
